New Client Intake Form

For NEW customers, in preparation for your appointment, you may download the "New Client Intake Form"and print it from your computer/printer.

From there you can fill out the needed information and bring it with you to the office at your next scheduled visit.

While copies of this form are also available at our office, you are urged to fill out this form before you see your massage therapist to expedite your appointment. 

If you are unable to print off this form, please plan to arrive earlier in order to fill out the paperwork.
